If you've been researching platelet-rich fibrin treatments, you've probably noticed that "PRF" isn't one-size-fits-all. Two forms, PRF EZ Gel and Liquid PRF, are used at Evolve Med Spa for different goals and different areas of the face and scalp. Both start with your own blood and harness the power of your body's natural growth factors. But the way they're prepared, delivered, and experienced is meaningfully different.

This guide breaks down exactly how each form of PRF works, what makes EZ Gel a unique advancement over traditional liquid PRF, and how to think about which one might be right for your skin or hair goals. (For a full overview of PRF treatments at Evolve, including candidacy, safety, and aftercare, visit our dedicated PRF service page.)

What Actually Happens Inside a Vial of PRF

Platelet-rich fibrin is made from your own blood, drawn in-office, just like a routine lab draw. The difference is what happens next. Your blood is placed in a centrifuge and spun at a carefully controlled speed. Unlike older platelet therapies, PRF is spun at a lower speed without anticoagulants, which preserves the fibrin matrix, a three-dimensional scaffold that holds platelets, white blood cells, and growth factors together.

That fibrin network is what makes PRF genuinely different from its predecessor, PRP (platelet-rich plasma). PRP releases growth factors quickly, like opening a floodgate. PRF releases them gradually, over days to weeks, because the growth factors are embedded in the fibrin mesh. The result is a more sustained regenerative signal to your tissue.

Key growth factors concentrated in PRF include PDGF (platelet-derived growth factor), VEGF (vascular endothelial growth factor), TGF-β, and IGF, all involved in collagen production, cellular repair, and new blood vessel formation.

Both EZ Gel and Liquid PRF start here. The centrifuge settings and tube preparation are what determine which form you get.

      Liquid PRF: Precise, Injectable, Targeted

Liquid PRF is spun at a slightly higher relative centrifugal force (RCF) and drawn from the upper-middle layer of the vial, the buffy coat zone just below the plasma. The result is a golden, fluid serum that can be loaded into a syringe and injected directly into tissue.

      What it's ideal for:

      • Under-eye hollows and the tear trough area
      • Fine lines around the lips and forehead
      • Scalp injections for hair thinning or hair loss
      • Microneedling channels (applied topically post-needling for deeper absorption)
      • Areas where precise, targeted delivery matters more than volume

Because Liquid PRF is fluid, it distributes evenly through tissue and begins releasing growth factors relatively quickly. It works with, not instead of, your body's existing repair mechanisms, which is why results build over time rather than appearing overnight.

For hair restoration specifically, Liquid PRF injected into the scalp reaches hair follicles in their resting (telogen) phase and helps signal them back into active growth. Multiple sessions spaced 4–6 weeks apart are typically recommended for meaningful hair density improvements.

PRF EZ Gel: The Advancement That Changed the Game

PRF EZ Gel takes the regenerative concept further by adding a thermal activation step. After centrifugation, the PRF is gently heated, typically to around 75–80°C, which causes the fibrin proteins to polymerize and transform from a liquid into a gel-like consistency. No synthetic additives. No cross-linking agents. Just your own biology, concentrated and converted.

The resulting gel has a smooth, cohesive texture that behaves similarly to a soft hyaluronic acid filler. It can be injected with a cannula or needle into areas where traditional liquid PRF would simply disperse, giving it volumizing and structural properties that liquid PRF cannot achieve on its own.

      What makes EZ Gel distinct:

      • Volume, it physically restores lost facial structure, not just stimulates it
      • Slower, more sustained growth factor release due to the denser fibrin matrix
      • 100% autologous, zero risk of allergic reaction or rejection
      • Completely bioabsorbable, the gel gradually integrates with your tissue over weeks
      • Compatible as a "booster" layered alongside dermal fillers
        • Because EZ Gel is made from your own blood, there's no allergy testing required and no waiting period between sessions for sensitivity concerns. The body recognizes and integrates the material naturally.

      Where EZ Gel excels:

      • Mid-face and cheek volume loss
      • Nasolabial folds and marionette lines (as an alternative or complement to filler)
      • Lip enhancement with a natural, soft result
      • Under-eye hollowing where both volume and skin quality are concerns
      • Temple hollowing
      • Jawline softening or definition

The dual action, immediate structural correction plus long-term biological stimulation, is what separates EZ Gel from both traditional filler and basic PRF. As the gel is absorbed over 4–8 weeks, the area has been flooded with growth factors the entire time, stimulating new collagen and elastin while simultaneously restoring lost volume.

What to Expect at Your PRF Appointment

Whether you're having EZ Gel or Liquid PRF, the appointment begins the same way: a small blood draw, typically 2–4 tubes depending on the treatment area and your provider's protocol. That blood goes directly into the centrifuge, the processing takes about 12–15 minutes.

For EZ Gel, there's an additional thermal activation step. The spun tubes are placed in a heating device for a few minutes to convert the PRF into gel form. You'll likely see this happening at the treatment station, the transformation from liquid to gel is visible.

      During the procedure:

      • A topical numbing cream is applied to the treatment area while your blood processes
      • EZ Gel is typically delivered via cannula (a blunt-tipped microcannula), which reduces bruising risk versus a sharp needle
      • Liquid PRF for the scalp involves a series of small superficial injections, a fine-gauge needle, spaced across the treatment zone
      • The actual injection time is usually 15–30 minutes depending on the area

Most patients describe PRF treatments as very tolerable, typically a 2–3 out of 10 on discomfort with numbing. Some swelling and tenderness is normal immediately after, especially with EZ Gel injections into the mid-face.

      Immediately after:

      • Mild swelling, redness, or bruising is expected and typically resolves within 2–5 days
      • EZ Gel patients may notice subtle volume correction almost immediately, this represents the physical gel before absorption begins
      • That initial volume will soften and partially reduce as the gel integrates, then gradually improve as collagen remodeling continues

The "dip" between weeks 2–4 is normal for EZ Gel patients: the initial gel volume softens before the biological regeneration catches up. Final results are typically visible at 6–8 weeks post-treatment.

PRF Results: What Happens Week by Week

      Liquid PRF (Skin / Under-eye):

      • Week 1–2: Injection sites heal; subtle improvement in skin hydration
      • Week 3–5: Growth factors actively stimulating fibroblast activity
      • Week 6–10: Visible improvement in fine lines, skin texture, and radiance
      • 3–6 months: Full collagen remodeling effect; results continue to build

      Liquid PRF (Scalp / Hair):

      • Month 1: Reduced shedding often reported first
      • Month 2–3: Early regrowth may appear as fine "baby hairs" at the hairline
      • Month 4–6: Increased density and hair shaft thickness become visible
      • Ongoing: Maintenance sessions every 6–12 months to sustain results

      PRF EZ Gel (Face):

      • Day 1–3: Swelling peaks then begins to subside; initial volume visible
      • Week 2–4: Gel integrating, slight volume reduction, normal and expected
      • Week 5–8: Collagen stimulation producing visible skin quality improvements
      • Month 2–3: Final result with improved structure, texture, and tone
      • Month 6–12: Results gradually absorb; touch-up or repeat treatment as desired

Unlike synthetic fillers, PRF EZ Gel doesn't require enzymatic dissolution if you're unhappy with placement, it resorbs naturally. And unlike filler alone, the biological stimulation component means your skin genuinely looks healthier, not just fuller.

Which Form of PRF Is Right for You?

The honest answer: many patients benefit from both, used for different purposes. EZ Gel and Liquid PRF aren't competing treatments, they're complementary tools in a regenerative aesthetics plan.

      Consider PRF EZ Gel if:

      • You've noticed volume loss in the cheeks, under-eyes, or temples
      • You want a natural alternative or complement to hyaluronic acid fillers
      • You're looking for visible correction plus long-term biological improvement
      • You want zero chance of foreign-body reaction or allergy

      Consider Liquid PRF if:

      • Your primary concern is hair thinning or early hair loss
      • You're targeting fine lines and skin texture without volume loss
      • You're combining with microneedling for a skin-quality treatment
      • You want to start with a lower-intensity regenerative option

At Evolve Med Spa, your provider will assess your anatomy, goals, and timeline to recommend the right combination. Many patients start with Liquid PRF and add EZ Gel as part of a full regenerative facial plan.
